ABSTRACT

RESUMEN Introducción: El Draf III es una técnica quirúrgica que permite el abordaje endoscópico de los senos frontales. Fue descrita como un procedimiento de rescate para la rinosinusitis crónica (RSCr) frontal en que falla la cirugía estándar. Actualmente, su uso se ha extendido a otras aplicaciones. Objetivo: (1) Revisar las indicaciones del abordaje Draf III, (2) Revisar si el abordaje permitió el manejo adecuado de la patología y (3) Evaluar la permeabilidad de los senos frontales. Material y método: Estudio retrospectivo descriptivo. Se evaluaron los registros de pacientes sometidos a un abordaje Draf III entre los años 2013-2016 (n =11). Se revisaron las indicaciones quirúrgicas, utilidad del Draf III para manejar la patología, permeabilidad de los senos frontales y complicaciones quirúrgicas. Resultados: Indicaciones quirúrgicas incluyeron la RSCr con pólipos, osteoma frontoetmoidal, mucocele frontal, papiloma invertido y estesioneuroblastoma. En todos los casos, el abordaje permitió un adecuado manejo de la patología. Todos los pacientes presentaron permeabilidad adecuada del Draf III al último control. Un paciente debió ser reoperado a los 15,5 meses por recidiva de papiloma invertido. Ningún paciente requirió cirugía de revisión por estenosis crítica del Draf III. No hubo complicaciones quirúrgicas. Discusión: El Draf III fue útil para manejar la RSCr, patologías benignas del seno frontal, y como parte de un abordaje extendido a la fosa craneal anterior. Conclusión: En nuestra serie, el Draf III resultó ser una excelente opción quirúrgica para el tratamiento de diversas patologías del seno frontal.


ABSTRACT Introduction: The Draf III procedure is an effective approach for the management of frontal sinus disorders. It was originally described as a rescue procedure for the treatment of refractory frontal sinusitis. Currently, it can be used to treat a variety of other disorders. Aim: (1) To review the indications for the Draf III procedure, (2) To evaluate if it enabled appropriate management of the disease, and (3) To evaluate frontal sinus patency. Material and Method: Retrospective descriptive study. Clinical records of patients who underwent a Draf III procedure between the years 2013-2016 (n=11) were reviewed. Surgical indications, the appropriateness of the approach to manage the frontal sinus disease, frontal sinus patency, and complications were analyzed. Results: Surgical indications included chronic rhinosinusitis with polyps, frontoethmoidal osteoma, frontal mucocele, inverted papilloma and esthesioneuroblastoma. In all cases, the approach allowed adequate management of the pathology. All patients had adequate patency of the Draf III at their last follow-up. One patient required a revision Draf III at 15.5 months follow-up because of an inverted papilloma recurrence. No patient required revision surgery for critical stenosis of the Draf III. No surgical complications were observed. Discussion: The Draf III procedure was useful for the management of chronic rhinosinusitis, for benign frontal sinus pathologies, and as part of an extended approach to the anterior cranial fossa. Conclusion: In our series, the Draf III procedure was an excellent surgical option for the management of a variety of frontal sinus pathologies.

ABSTRACT

Los osteomas fronto-etmoidales son los tumores benignos más frecuentes de los senos paranasales, pudiendo evolucionar con complicaciones por compresión de estructuras adyacentes. Se presenta el caso de una paciente de 63 años de edad, que consultó por aumento de volumen nasofrontal, cefalea y diplopía. Tras el examen clínico, se evidenció una asimetría facial con lateralización del bulbo ocular derecho y exoftalmo. La tomografía de los senos paranasales mostró una lesión que ocupa parcialmente el seno frontal, etmoidal y parte de la cavidad nasal. Los cuidados anatómicos y planificación quirúrgica se desarrollaron en un modelo estereolitográfico mientras que la cirugía de exéresis total se realizó con ayuda del sistema piezoeléctrico. El examen histológico confirmó el diagnostico de osteoma. Se obtuvo una reconstrucción exitosa, recuperando totalmente simetría y función ocular.


Fronto-ethmoidal osteomas are the most frequent benign tumors of the paranasal sinuses, and may evolve with complications by compression of adjacent structures. The case is a 63-year-old female patient, who consulted about increased nasofrontal volume, headache and diplopia. After the clinical exam, she presented a facial asymmetry with lateralization of the right eyeball and exophthalmus. Computed tomography of the sinuses showed a lesion that partially occupies the frontal sinus, ethmoidal sinus and part of the nasal cavity. The anatomical care and surgical planning was developed in a stereolithographic model while the total excision surgery was performed with the help of the piezoelectric system. Histological examination confirmed the diagnosis of osteoma. A successful reconstruction is obtained, fully recovering symmetry and ocular function.

ABSTRACT

La celdilla supra-órbito-etmoidal es una variación anatómica que se presenta con diferente frecuencia para las distintas poblaciones. Representa un desafío al realizar cirugía endoscópica funcional del seno frontal debido a que su presencia dificulta el drenaje adecuado del receso del frontal mismo y puede ser confundida con el seno. La celdilla supra-órbito-etmoidal presenta relaciones espaciales constantes con la arteria etmoidal anterior, sirviendo como marcador anatómico confiable para el abordaje quirúrgico. Se destaca la importancia de considerar la presencia de tabiques óseos en el seno frontal en el estudio imagenológico que puedan sugerir la presencia de la celdilla supra-órbito-etmoidal. Se presenta el caso clínico de un paciente que consulta por proptosis del ojo derecho debido a un proceso expansivo por mucocele localizado en dicha variante anatómica.


The supraorbital ethmoid cell its an anatomical variation with different frecuency for diverse populations. It represents a challenge for the endoscopical functional :surgery of the frontal sinus. The supraorbital ethmoid cell presents an estable anatomical relationship with the anterior ethmoidal artery, serving as a dependable anatomical marker for the surgical approach. We stand out the importance to consider the presence of bony septations in the frontal sinus that might suggest the presence of the supraorbital ethmoid cell. We introduce a clinical case from a patient who had an expansive process from a mucocele in the supraorbital ethmoid cell.

ABSTRACT

Presentamos el caso de una mujer de 57 años, que consulta tras notar un aumento de volumen pétreo y de lento crecimiento en el reborde orbitario medial derecho, asociado a obstrucción nasal, anosmia y descarga posterior. La evaluación clínico radiológica evidenció un gran osteoma frontoetmoidal derecho con compromiso orbitario y de la lamela lateral de la lámina cribosa, y además, una rinosinusitis crónica con pólipos. Se realizó una resección endoscópica del tumor por medio de un Draf III y una cirugía endoscópica funcional del resto de las cavidades perinasales para el tratamiento de la rinosinusitis crónica. Describimos el caso y discutimos sus aspectos relevantes.


We report the case of a 57 year-old woman consulting because of a slow growing solid tumor in the right medial orbital rim associated with nasal obstruction, anosmia and posterior nasal drip. Clinical and radiological evaluation evidenced a large right frontoethmoidal osteoma involving the orbit and thelateral lamella of the cribriform plate, and chronic rhinosinusitis with polyps. A Draf III was performed to remove the osteoma and a bilateral functional endoscopic sinus surgery of the remaining sinuses was performed to treat thechronic rhinosinusitis. We described the case and discuss its relevant aspects.

ABSTRACT

El tratamiento endoscópico de las fístulas de LCR se ha convertido en el gold standard. No obstante, aquellas localizadas en seno frontal presentan serias dificultades para su abordaje endoscópico, especialmente si se asocia la existencia de un meningoencefalocele. En estos casos se debe valorar el abordaje externo con obliteración del seno frontal. Presentamos dos casos de fístulas de LCR cuyo origen fue debido a sendos meningoencefaloceles protruyendo a través de la pared posterior del seno frontal y que precisaron de un abordaje externo para su tratamiento.


Endoscopic treatment of CSF leak is the Gold Standard. Nevertheless, at frontal sinus it is very difficult the endoscopic approach, especially if there is a meningoencephalocele associated. In these cases, an external approach with frontal sinus obliteration should be considered. We present two cases of CSF leaks whose origin are meningoencephaloceles that protrude from posterior frontal sinus wall, treated with an external approach.

ABSTRACT

Primary frontal sinus lymphoma is a very uncommon disease. In all the previously reported cases, the presenting symptoms have been due to the tumor mass effect. We present an unusual case report of an immunocompetent patient who presented with facial palsy, and then progressively developed other cranial nerve palsies over several months. He was later diagnosed with diffuse large B cell lymphoma originating from the frontal sinus. The patient underwent chemotherapy, but eventually had to receive autologous peripheral blood stem cell transplantation. He is currently disease-free. The clinical course, diagnostic workup, and therapeutic outcome are described.

ABSTRACT

Psammomatoid ossifying fibroma is a rare slowly progressive tumour of the extragnathic craniofacial bones, with a tendency toward locally aggressive behaviour. The pathognomonic histologic feature is the presence of spherical ossicles which are similar to psammoma bodies. A fibro-osseous lesion composed of psammomatoid ossifying fibroma and aneurysmal bone cyst of the frontal sinus in a 12-year-old boy is reported, followed by a literature review. To our knowledge, this is the second case of a hybrid fibro-osseous lesion composed of ossifying fibroma and aneurysmal bone cyst to be reported in the literature

ABSTRACT

A 31-year-old lady presented to the otorhinolaryngology clinic complaining of right exophtalmus. Imaging studies revealed a right frontal sinus mass, suggestive of a mucocele, and fronto-ethmoidal ground glass opacification. In the operating room, the frontal sinuses were exposed through an osteoplastic flap, and connected to the nasal cavity through a Lothrop-like drainage pathway. The pathologic analysis revealed a frontal sinus mucocele and fibro-osseous and respiratory epithelial hamartoma of the fronto-ethmoid region. In this case report, this rare pathology is discussed, in addition to a thorough elaboration upon the surgical options as dictated by the anatomic complexity of the region

Cementifying ossifying fibroma is a fibro-osseous lesion which produces cementum. Cementifying ossifying fibroma is a mesodermal, non-odontogenic tumor of ectopic multipotential periodontal membrane blast cells. Although, it is a benign bone tumor, it has an aggressive destructive behavior in patients under 20 years of age with high tendency for recurrence. Cementifying ossifying fibroma can be recognized microscopically by the presence of cementicles. This is a case of frontal sinus cementifying ossifying fibroma presenting with proptosis in a 39-year-old male

ABSTRACT

Se realizó un estudio retrospectivo de los pacientes intervenidos quirúrgicamente por mucoceles de los senos paranasales en el periodo comprendido de mayo de 1984 a diciembre de 1997. Se analizaron sus causas, evolución y sobre todo los resultados al tratamiento con seguimiento a mediano y largo plazo. Se efectuaron 263 procedimientos quirúrgicos por patología inflamatoria nasosinusal, de las cuales el 6.4 por ciento (17 pacientes) fue por mucoceles. La edad de presentación fue en promedio de 38.6 años, con localización frontal en el 41.1 por ciento, y frontoetmoidal en 17.6 por ciento. El síntoma más frecuentemente encontrado fue aumento de volumen, y el manejo quirúrgico fue por vía externa y/o endonasal microscópica en 15 pacientes, y en 2 enfermos el procedimiento fue mediante cirugía endoscópica. El seguimiento varió de 4 a 108 meses con media de 37.5 meses, obteniendo resultados favorables en todos los casos con mínimas complicaciones.

ABSTRACT

Los tumores pardos son lesiones óseas focales secundarias a hiperparatiroidismo. Se han descrito en la literatura 7 casos asociados a hiperparatiroidismo secundario localizados en hueso frontal, maxilar y etmoidal que involucran la órbita. Se presenta un caso clínico, en una mujer de 58 años de edad con insuficiencia crónica de 18 años de evolución; en diálisis peritoneal crónica ambulatoria y en programa de transplante renal. Seis meses antes inicia aumento de volumen inter ciliar de predominio izquierdo. presenta desplazamiento ocular izquierdo por tumor de aproximadamente 10 x 7 cm, fijo, no doloroso y sin cambios tróficos en la piel supra yacente. Se extirpó el tumor que fue diagnosticado como "tumor pardo"

ABSTRACT

A Seventeen years old male presented with right unilateral proptosis and supra-orbital swelling. Routine x-rays sinuses and CT Scan showed opacified mass of the right frontal sinus mainly occupying the lateral part of the sinus extending into the orbit. Excision of the lesion was carried out under general anaesthesia utilizing unilateral osteoplastic flap. The procedure was devised to ensure complete excision of osteoma through frontal sinus, direct examination of the frontonasal duct opening, and to achieve a good post operative cosmesis

ABSTRACT

A patients with a fronto-ethmoidal mucocele secondary to fibromatosis of the nose and paranasal air sinuses is presented. The case was operated upon for first time in 1980 to remove a right fronto-ethmoidal mucocele, when a fibrous firm tissue mass was excised from the right side of his nasal cavity at the frontonasal opening. This mass was incorrectly diagnosed as fibrosarcoma. Fortunately radiotherapy was not given for fear of affecting vision. Frequent recurrences of the mucocoele with infection was treated medically and surgically. The closure of the frontonasal duct and the right upper part of nasal cavity with new bone and fibrous tissue, and the long standing condition of the patient aroused suspicion regarding the pathological diagnosis. Review of previous histopathology and fresh specimen proved that this is a case of fibromatosis of the paranasal sinuses
